The Department of State Hospitals is accepting applications for the Board of Parole Hearings (BPH) Mentally Disordered Offender (MDO) Independent Evaluator Panel until July 1, 2017.
Psychiatrists and Psychologists interested in becoming independent evaluators for the BPH must submit a MDO Independent Evaluator Application and be approved by the Department of State Hospitals. For more information regarding independent evaluator recruitment, including where to submit your application, please visit the Department of State Hospitals website.
The BPH will hold mandatory orientation on BPH functions and MDO hearings in September 2017 as follows:

The Board of Parole Hearings invoice form to bill for services is available below.
IF YOU ARE WORKING WITH BPH FOR THE FIRST TIME AS AN INDEPENDENT EVALUATOR, you must also submit a Payee Data Record Form and a Vendor Master Record Information Form so you can be paid. Independent Evaluators already on record with BPH need not submit these forms.
