CSA Download Instructions

All Publications are available in their original format as a WORD 2000 document file which you can view, modify and print when you download it. To view and use the files you MUST have WORD 2000 installed on your system.

To download a form select it and save it as a word file. After this process you will be able to access this form and complete it in WORD 2000 as it pertains to your agency and mail to the Board of Corrections. You will be able to access the input fields by using the TAB key.
