Reports for Title II, V and Jail Removal Grant Recipients

In order to streamline the reporting process, we have modified the progress report and incorporated it with the quarterly invoices. Please begin using the new format immediately and submit through email with your draft quarterly invoice. This format should also be utilized for any reports that are due for the period covering January 1, 2004 through June 30, 2004 and for the quarterly report covering April 1, 2004 - June 30, 2004. We encourage you to complete all sections, including the requested statistical data for the past, but if unavailable please note and assure that steps will be taken to collect the requested data for the remaining report periods.

Beginning July 1, 2004, quarterly statistics regarding numbers of youth served must be submitted with the Financial Invoice (201).

Please take note of the questions and data that will be required for the final report. The form is available at TitleII Progress Report-Rev 7a2.doc.
