Transitions Clinic Network is Hiring

The Transitions Clinic Network is currently hiring two California-based positions. 1.     California Program Manager: Seeking skilled program manager with a fierce commitment to social justice to manage TCN’s statewide network of primary care clinics that serve individuals returning from incarceration. This position supports implementation of the TCN model of care and onboarding of new sites to the network. manages existing contracts and grants, leads program development, manages training and networking events, and, under leadership of TCN’s Executive Director, provides input on advocacy efforts and key statewide policies. This position is currently remote with possibility of returning to our office in San Francisco, CA. California-based applicants strongly preferred.  

2.     Community Health Worker (CHW) Trainer: Seeking skilled trainer who is passionate about building capacity among a network of CHWs with lived experience of incarceration. This staff will support TCN’s Lead CHW in training and capacity building efforts and developing and sustaining relationships with community-based organizations, advocates and allied organizations. They will also contribute to other TCN initiatives focused on improving the health and reentry of individuals returning from incarceration. This position requires lived experience of incarceration. This position is currently remote with possibility of returning to office in San Francisco, CA. California-based applicants only.
