{"id":1147,"date":"2023-11-21T15:49:45","date_gmt":"2023-11-21T23:49:45","guid":{"rendered":"https:\/\/www.cdcr.ca.gov\/hcdom\/?post_type=dom&#038;p=1147"},"modified":"2024-12-27T13:50:45","modified_gmt":"2024-12-27T21:50:45","slug":"3-5-19-medication-shortages-or-backorders","status":"publish","type":"dom","link":"https:\/\/www.cdcr.ca.gov\/hcdom\/dom\/chapter-3-health-care-operations\/article-5-pharmacy\/3-5-19-medication-shortages-or-backorders\/","title":{"rendered":"3.5.19 Medication Shortages or Backorders"},"content":{"rendered":"\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p><strong>Procedure Overview<\/strong><\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block is-style-no-marker\">\n\t\t\t\t\t<p>California Correctional Health Care Services shall maintain a system to ensure that prescribers and other health care professionals are informed when prescribed medications cannot be provided in a timely manner because of inventory shortages, supply system shortages, or backorders.\u00a0 The system shall include a process to allow prescribers to change therapy when appropriate and to notify health care professionals and patients of the change.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p><strong>Purpose<\/strong><\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block is-style-no-marker\">\n\t\t\t\t\t<p>To ensure that prescribers have the opportunity to assess the impact on patient therapy and adjust the care plan in a timely fashion when medications are unavailable due to supply system interruptions.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p><strong>Procedure<\/strong><\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>When it is determined that a prescribed medication is unavailable from the supplier because of an inventory shortage, manufacturer shortage, backorder, or other reasons, the Pharmacist-in-Charge, or pharmacist designee, shall assess the anticipated duration of delay.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The pharmacist shall communicate to the prescriber, or designee, and nursing staff when medication cannot be provided in the expected timeframe due to supply system interruptions.<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The communication shall include an anticipated timeframe for obtaining the medication.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The prescriber shall determine the appropriate response and inform the pharmacist.\u00a0 The provider shall prescribe an alternative medication or authorize a delay in medication delivery if the timeframe is safe and appropriate. The prescriber shall document the decision to prescribe an alternative medication or authorize a delay in the health record.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The pharmacist shall communicate the decision to nursing staff.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Nursing staff shall communicate the decision to the patient.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The pharmacy shall keep a log of medications affected by the supplier interruption. Every business day, until the shortage resolves, the pharmacy shall reassess the availability of the prescribed medication and alternatives.
