The CDC Form is an official document, and if changes are necessary, a new form shall be prepared.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Upon receipt of court ordered pre-confinement credits received after the BPT has signed the form, it is necessary to re-compute the DSL dates on a new CDC Form 678. Indicate &#8220;Refer to signed CDC Form 678 dated __________&#8221; on the new form, attach to corrected copy and file in permanent addenda.<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Should the application of such credit change the base term, return the case to the BPT for re-review and signature.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The new CDC Form 678 is to be labeled &#8220;recalculated&#8221; above the CCRM signature block.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<\/ul>\n\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Whenever the controlling release date changes that change will need to be reflected on the &#8220;Controlling Release Date __________&#8221; line.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Additionally, the &#8220;(as of __________)&#8221; will be recorded as of the date the change is made.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>When the CDC Form 678 is found to have an error, a new form shall be prepared and marked &#8220;corrected copy&#8221; above the CCRM signature block.<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Send a copy to the inmate.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Amend the information in OBIS to reflect the change(s).<\/p>\n\t\t\t\t\t<\/li>\n\t\t<\/ul>\n\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>If the BPT takes an action that changes the ISL data on the CDC Form 678, it is necessary to update the original CDC Form 678. When updating the ISL data, change the &#8220;as of __________&#8221; date to the date the change is made.
