The form must be submitted on or before the due date provided by CTP staff.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Within one business day of submission of the BPH Form 3005, the parole agent shall complete CDCR Form 611 and submit the form to the unit supervisor.
