Information shall never be recorded in this area when preparing a &#8220;New&#8221; CDC Form 839 score sheet.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>This area is not to be used for changes or adjustments to term points.
