{"id":7270,"date":"2020-03-31T09:23:12","date_gmt":"2020-03-31T16:23:12","guid":{"rendered":"http:\/\/cdcr.test\/dom\/chapter-1-general-administration\/article-15-information-practices\/13030-12-1-content-of-notice\/"},"modified":"2020-03-31T09:23:12","modified_gmt":"2020-03-31T16:23:12","slug":"13030-12-1-content-of-notice","status":"publish","type":"dom","link":"https:\/\/www.cdcr.ca.gov\/operations-manual\/dom\/chapter-1-general-administration\/article-15-information-practices\/13030-12-1-content-of-notice\/","title":{"rendered":"13030.12.1 Content of Notice"},"content":{"rendered":"\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The notice shall cover all of the following:<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The CDCR name.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The title, business address, and telephone number of CDCR official who is responsible for the system of records and who shall, upon request, inform an individual regarding the location of his or her records and the categories of any persons who use the information in those records.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The authority, whether granted by statute, regulation, or executive order, which authorizes the maintenance of the information.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>With respect to each item of information, whether submission of such information is mandatory or voluntary.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The consequences, if any, of not providing all or any part of the requested information.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The principal purpose or purposes within CDCR for which the information shall be used.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Any known or reasonably foreseeable disclosure which may be made of the information.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The individual&#8217;s right to inspect records containing personal information which is maintained by CDCR.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<\/ul>\n\t\t<\/li>\n\t\t<\/ul>\n\t","protected":false},"parent":7256,"template":"","class_list":["post-7270","dom","type-dom","status-publish","hentry"],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.3 -
