A claim submitted by an administration or field office employee shall be approved by the employee\u2019s immediate supervisor and the appropriate division\/department head or designee.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>All claims involving reimbursement of stolen personal tools or equipment valued in excess of $500 must be approved by CalHR.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>All claims shall be submitted using CDCR Form 892, Employee Claim for Loss or Damage to Personal Property.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>All claims shall include the employee\u2019s verification of the following on CDCR Form 892:<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The item was used in the course of employment<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Loss or damage of the item occurred at the employee\u2019s work site, work base, or en route between the two locations<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Loss or damage of the item was not a result of carelessness or negligence Additional documentation required for damaged personal property:<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>If the item is repairable, provide a researched estimate of the cost for any repairs<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>If the item is irreparable, provide the reason(s) why the item cannot be repaired and evidence of the item\u2019s value at the time of damage, e.g., sales records, price lists, etc.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<\/ul>\n\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Additional documentation required for stolen personal tools or equipment:<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The official CHP report describing the incident or the circumstances of the theft<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Evidence of the item\u2019s value at the time of theft, e.g., sales records, price lists, etc.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<\/ul>\n\t\t<\/li>\n\t\t<\/ul>\n\t","protected":false},"parent":7904,"template":"","class_list":["post-7909","dom","type-dom","status-publish","hentry"],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.8 -
