{"id":8318,"date":"2020-03-31T09:24:28","date_gmt":"2020-03-31T16:24:28","guid":{"rendered":"http:\/\/cdcr.test\/dom\/chapter-3-personnel-training-and-employee-relations\/article-7-personnel-identification-cards\/31070-7-lost-cards\/"},"modified":"2024-10-29T23:08:11","modified_gmt":"2024-10-29T23:08:11","slug":"31070-7-lost-cards","status":"publish","type":"dom","link":"https:\/\/www.cdcr.ca.gov\/operations-manual\/dom\/chapter-3-personnel-training-and-employee-relations\/article-7-personnel-identification-cards\/31070-7-lost-cards\/","title":{"rendered":"31070.7 Lost Departmental Identification Cards"},"content":{"rendered":"\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Any individual who loses the departmental identification (ID) card shall immediately report the loss by completing a Lost ID Badge form located on the Human Resources (HR) intranet site <a href=\"https:\/\/cdcr.sharepoint.com\/sites\/cdcr_hub_hr\/SitePages\/Lost-ID-Portal.aspx\">http:\/\/intranet\/ADM\/DSS\/hr\/ops\/Pages\/Lost%20ID.aspx<\/a>.<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The completed form is to be signed by a supervisor and submitted to the<br>hiring authority (HA).<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The HA will review the form and provide the signed Lost ID form to the<br>Lost ID Coordinator.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Each program is allowed two Lost ID Coordinators.<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The staff identified by the HA as Lost ID Coordinators will be provided access to the Department\u2019s HR intranet Lost ID site.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>If there are no Lost ID Coordinators, the HA shall click the \u201cRequest access to post Lost ID\u201d link located on the Department\u2019s HR intranet Lost ID page. The link will open an email to CDCR HR Personnel Customer Service.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The facility, parole region, or division Lost ID Coordinator, or HA if no Lost ID Coordinators are identified, shall:<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Post the information on the Department\u2019s HR intranet Lost ID page list to advise all departmental staff that the ID card has been reported lost.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Send a notice to all other facilities advising of the loss.
