{"id":8573,"date":"2020-03-31T09:24:37","date_gmt":"2020-03-31T16:24:37","guid":{"rendered":"http:\/\/cdcr.test\/dom\/chapter-3-personnel-training-and-employee-relations\/article-18-general-training\/32010-8-3-record-keeping-forms\/"},"modified":"2026-04-02T17:15:34","modified_gmt":"2026-04-02T17:15:34","slug":"32010-8-3-record-keeping-forms","status":"publish","type":"dom","link":"https:\/\/www.cdcr.ca.gov\/operations-manual\/dom\/chapter-3-personnel-training-and-employee-relations\/article-18-general-training\/32010-8-3-record-keeping-forms\/","title":{"rendered":"32010.8.3 Record Keeping Forms"},"content":{"rendered":"\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The following forms shall be used to record training requests and participation:<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>CDCR Form 844, Training Participation Sign-In Sheet.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>No more than one course of instruction shall be recorded on a CDCR Form 844. Any CDCR Form 844 with more than one subject should not be accepted by the division\/unit training office.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The CDCR Form 844 shall not be required if training attendance is directly recorded utilizing the departmentally approved electronic tracking system.<\/p>\n\t\t\t\t\t<\/li>\n\t\n\t<\/ul>\n\t\n\t<\/li>\n\t\n\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>CDCR Form 854, Employee Training Request.<\/p>\n\t\t\t\t\n\t<ul class=\"cdcr-dom-group-block\">\n\t\t\n\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Use is required only if the requested training falls outside the approved annual training plan or annual training requirements.
