{"id":8818,"date":"2020-03-31T09:24:44","date_gmt":"2020-03-31T16:24:44","guid":{"rendered":"http:\/\/cdcr.test\/dom\/chapter-3-personnel-training-and-employee-relations\/article-22-employee-discipline\/33030-23-1-documentation-format\/"},"modified":"2020-03-31T09:24:44","modified_gmt":"2020-03-31T16:24:44","slug":"33030-23-1-documentation-format","status":"publish","type":"dom","link":"https:\/\/www.cdcr.ca.gov\/operations-manual\/dom\/chapter-3-personnel-training-and-employee-relations\/article-22-employee-discipline\/33030-23-1-documentation-format\/","title":{"rendered":"33030.23.1 Documentation Format"},"content":{"rendered":"\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Clerical support staff, under the supervision of the ERO\/Disciplinary Officer, shall compile adverse action documentation as follows:<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Type the Preliminary Notice of Action on the departmental form memorandum.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>Type Notice of Adverse Action on Department letterhead as follows:<\/p>\n\t\t\t\t\t<ul class=\"cdcr-dom-group-block\">\n\t\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The type of notice shall appear in capital letters and shall be centered four spaces below the letterhead.<\/p>\n\t\t\t\t\t<\/li>\n\t\t<li class=\"cdcr-dom-item-block\">\n\t\t\t\t\t<p>The employee&#8217;s name, civil service classification, worksite name and location (including institution or parole region, if applicable) are typed in block style at the left-hand margin, four spaces below the title.
