{"id":497,"date":"2019-06-09T04:11:02","date_gmt":"2019-06-09T04:11:02","guid":{"rendered":"http:\/\/cdcr-migration.10upmanaged.com\/parole\/mental-health-services-continuum-program\/"},"modified":"2026-03-26T14:12:31","modified_gmt":"2026-03-26T21:12:31","slug":"mental-health-services-continuum-program","status":"publish","type":"page","link":"https:\/\/www.cdcr.ca.gov\/parole\/mental-health-services-continuum-program\/","title":{"rendered":"Behavioral Health Reintegration (BHR)"},"content":{"rendered":"\n<p>Behavioral Health Reintegration (BHR) is a first line resource of clinical case management services designed to assist parolees with their successful reintegration back into the community.&nbsp;&nbsp;Located in the parole offices,&nbsp;BHR is staffed with psychiatrists, psychologists, and social workers who have the unique understanding that, while in prison, inmates have their core basic needs provided for them.&nbsp; Once paroled, these individuals are responsible for navigating their own resources in obtaining food, clothing, shelter, and medical\/mental health care.&nbsp; Without focused transitional re-entry support, the daily stress from these new realities can contribute to the triggering of symptoms, increase risk of substance use and criminogenic behaviors.&nbsp; BHR clinicians understand that providing effective clinical and case management interventions in the community, versus in a controlled environment, necessitates a broader psychosocial approach accounting for the fluctuation of symptom, functional abilities, need acuity, readiness to change and service availability.&nbsp; Parolees require individualized assessments and functional treatment plans to address the dynamic needs and obstacles that post incarceration life presents.&nbsp; \u200b<\/p>\n\n\n\n<p>BHR services focus on establishing detailed and individualized assessment of parolee needs while establishing short term and long-term plans to meet identified goals and objectives.&nbsp; Establishing sustainable long-term community-based services and resources that will sustain once discharged from parole supervision is the overarching goal.&nbsp; &nbsp;BHR clinicians are continuously available to provide brief clinical intervention as well as group and family systems support services.<\/p>\n\n\n\n<p>Methods of services BHR provides include, but are not limited to, evaluation of mental illness and substance use, on-going assessment of symptom severity and intervention, , medication management, Medicated Assisted Treatment (MAT), brief individual therapy, group therapy, crisis intervention, case management support and field-based services designed to engage higher need clients in the community.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Medical Records Requests:<\/h2>\n\n\n\n<p>For records related to Mental Health Services provided to supervised persons, please submit a completed Authorization for Release of Protected Health Information&nbsp;<a href=\"https:\/\/cdcr.sharepoint.com\/:b:\/r\/sites\/cdcr_hub\/Documents\/CDCR%207385%20-%20AUTHORIZATION%20FOR%20RELEASE%20OF%20PROTECTED%20HEALTH%20INFORMATION.pdf?csf=1&amp;web=1&amp;e=d9tUc1\">CDCR 7385<\/a>&nbsp;using one of<br>the following methods, listed in order of preference:<br><br><strong>Email (preferred for fastest processing \u2013 all regions):<\/strong><br><br>Send the Release of Information (ROI) to:&nbsp;<a href=\"https:\/\/cdcr.ca.gov.\/\">m_BHRRecordRequests@cdcr.ca.gov.<\/a><br><strong><em>Note: This e-mail address is for medical records requests only.<\/em><\/strong><\/p>\n\n\n\n<p><br><strong>Fax (all regions):<\/strong><br>(279) 223-7909<\/p>\n\n\n\n<p><br><strong>Mail (choose the appropriate regional address):<\/strong><br>1.
