Note to Out of State Applicants

• Applicants currently living out of California or that lived out of California in the past five (5) years must provide their own driving record history from the respective Department of Motor Vehicles (DMV). Applicants are required to provide their past five (5) year driving record history.
• Out of State applicants will be assigned to one of the following selection centers which can be changed by contacting OPOS, Selection Support Section.
• Be prepared to turn in your Personal History Statement (PHS), Background Investigation Questionnaire (BIQ) and any other documents necessary when you come in for testing.
