Attorney Invoicing
The Board of Parole Hearings (BPH) invoice forms to bill for services are available below. The BPH 1076 and BPH 1076(A) – DNV/DPV/DPH invoices are used for suitability hearings and the BPH 1076-OMHD invoice is used for Offenders with Mental Health Disorder proceedings. For suitability hearings, please see the notes next to each invoice link regarding which invoice version applies to your specific case.
- BPH 1076: Attorney Invoice (rev 1-2025) – for cases accepted on or after July 1, 2024 (except for DNV, DPV, or DPH clients)
- BPH 1076(A): DNV_DPV_DPH Attorney Invoice (rev 1-17-2025) – for cases accepted on or after July 1, 2024, for DNV, DPV, or DPH clients
- BPH 1076: Attorney Invoice (rev 08-23-2023) – for all cases accepted before June 30, 2024
- BPH 1076: OMHD Attorney Invoice (rev 1-2022) – for hearings on or after January 1, 2020
IF YOU ARE WORKING WITH BPH FOR THE FIRST TIME AS AN ATTORNEY, you must also submit a Payee Data Record Form and a Supplement Vendor Payee Data Record Form so you can be paid. Attorneys already on record with BPH need not submit these forms.
