The Board of Parole Hearings (BPH) invoice forms to bill for services are available below. The BPH 1076 invoice is used for suitability hearings and the BPH 1076 invoice is used for Offender Mental Health Disorder hearings.
- BPH 1076: Attorney Invoice (rev 8-2018) –for hearings on or before December 31, 2019
- BPH 1076: Attorney Invoice (rev 9-2019)– for hearings on or after January 1, 2020
The Board will continue assigning current active attorneys under the existing Panel Attorney Program to inmate clients who have hearings scheduled in the first few months of 2020. Attorneys will receive the increased reimbursement rate for hearings scheduled after January 1, 2020, upon certifying they have complied with the panel attorney expectations as described in the new Panel Attorney Program Guide.

IF YOU ARE WORKING WITH BPH FOR THE FIRST TIME AS AN ATTORNEY, you must also submit a Payee Data Record Form and a Supplement Vendor Payee Data Record Form so you can be paid. Attorneys already on record with BPH need not submit these forms.
