The Department of Health Care Services (DHCS) recently posted the fillable application template for Round 1 of the Providing Access and Transforming Health (PATH) Justice-Involved Capacity Building Program on the DHCS CalAIM Justice-Involved Initiatives webpage. View the application template.
Please note, the questions in the fillable application template are identical to those in the non-fillable version that was shared with stakeholders on May 13, 2022. Eligible applicants should use the fillable pdf application template when applying for Round 1 funding. Eligible entities will be able to apply for funding through this initiative from now until July 31st.
Additional materials related to the PATH Justice-Involved Capacity Building Program will be disseminated via DHCS General Stakeholder Updates and will also be posted to the CalAIM Justice-Involved website.
