Task Statements:

1. Recommend training on re-entry issues for discharge to the treatment team staff to increase their knowledge of re-entry functions using re-entry planning documents, etc., in accordance with the laws, rules/regulations, and Division of Juvenile Justice (DJJ) policies/procedures.

3. Develop materials for internal and external stakeholders by incorporating the principles of the Integrated Behavior Treatment Model (IBTM) and reentry planning in accordance with the laws rules/regulations, and DJJ polices/procedures.

4. Provide overviews for internal and external stakeholders by incorporating the principles of the IBTM and reentry planning in accordance with the laws, rules/regulations, and DJJ policies/procedures.

6. Facilitate staff training to increase knowledge and skills related to the principles of the IBTM and re-entry planning in accordance with DJJ policies/procedures.

7. Complete on the job training of facility staff to increase knowledge and skills related to the principles of the IBTM and re-entry planning in accordance with DJJ policies/procedures.

8. Provide coaching of facility staff to increase knowledge and skills related to the principles of the IBTM and re-entry planning in accordance with DJJ policies/procedures.

9. Participate with the multi-disciplinary team to create a re-entry plan incorporating risk/needs by using the IBTM for successful integration to the community in accordance with DJJ polices/procedures.

10. Monitor compliance of the re-entry plan development and incorporating risk/needs by using the IBTM for successful integration to the community in accordance with DJJ policies/procedures.

11. Provide support and technical assistance to facility staff to ensure youth are appropriately placed according to the California Youth Assessment Screening Instrument (CA-YASI) risk level in accordance with DJJ policies/procedures.

12. Operate a state vehicle when attending/facilitating meetings/trainings to disseminate information relevant to DJJ programs and/or acting as a liaison between law enforcement and public agencies in accordance with the DJJ policies/procedures.

13. Monitor the progress of the case plan to identify and address the youth’s re-entry needs by utilizing the principles of the IBTM in accordance with DJJ polices/procedures.

14. Review the risk/needs assessment tool of the youth to develop a re-entry plan that incorporates the youth’s experience and training received (e.g., education, employment, and skills, etc.) utilizing the principles of the IBTM in accordance with DJJ polices/procedures.

15. Serve as a resource specialist to develop and maintain contacts for appropriate program utilization, integration, and accessibility by utilizing community-based providers and programs in accordance with the laws, regulations, and DJJ polices/procedures.

16. Collaborate with internal and external stakeholders involved in the development of the re-entry plan to assist and develop a youth’s continued community treatment needs (e.g., counseling, medication and health services, etc.) by utilizing community-based providers and programs in accordance with the laws, regulations, and DJJ polices/procedures.

17. Provides mandated information (e.g., current location, pending release, community adjustment, etc.) regarding youth to appropriate family members, law enforcement, state and county agencies by providing written documentation in accordance with the laws, regulations, and DJJ polices/procedures.

18. Serve as a resource specialist to assist the multi-disciplinary team and youth with identified treatment resources in the community in accordance with the laws, regulations, DJJ policies and procedures.

19. Monitor the progress of the youth’s re-entry needs to finalize all resources by incorporating the information into the youth’s re-entry plan (e.g., employment, educational and treatment resources, etc.,) in accordance with DJJ policies and procedures.

20. Collaborate and work with the multi-disciplinary team, state and county agencies on specialized or high risk cases to successfully integrate youth to the community (e.g., former foster youth, homeless and mental health, etc.,) utilizing available resources in accordance with the laws, regulations, and DJJ polices/procedures.

23. Promote involvement between youth, families, state and county agencies in order to improve relationships by utilizing facility visits, telephone conversations, face to face contacts, etc., in accordance with the laws, regulations, and DJJ polices/procedures.

25. Attend mandated training to keep informed of new policies and standards of the DJJ and Board of Juvenile Hearings (BJH) and maintain proficiency using on the job training, in-service training, videos, bulletins and lectures, etc., in accordance with the laws, regulations, DJJ policies/procedures, as required.

26. Assist the youth with re-integration into the community by referring the youth to the appropriate community resources and services (e.g., mental health, substance abuse, education, etc.,) in accordance with the laws, regulations, DJJ polices/procedures.

27. Assist the multi-disciplinary IBTM training team to support the principles of IBTM including policy development, training, coaching, and mentoring DJJ staff in accordance with the laws, regulations, DJJ polices/procedures.
