Recently Released: How to Share Data: A Practical Guide for Health and Homeless Systems of Care

In Fall 2021, Homebase hosted a webinar series addressing cross-sector collaboration between health and homeless systems of care. Homebase is a collective of legal, policy, and subject matter experts, skilled facilitators, and strategic thinkers and planners dedicated to addressing homelessness and its root causes. Based on the responses received to the webinar series, the Homelessness Response 101 overview, and Breaking Down Silos report, with the support of the California Health Care Foundation, they developed a follow up practical guide to support communities that are considering or in the process of cross-sector data sharing.

With California’s CalAIM efforts just beginning to take shape, there are ample opportunities for deeper cross-sector collaboration between health and homeless systems of care. “How to Share Data: A Practical Guide for Health and Homeless Systems of Care” is organized to provide recommended steps, key things to consider, and real-life examples reflecting different approaches to data sharing. The webinars from Fall 2021 are still available on the Homebase Health Care web page to watch for free, as well.
