31070.7 Lost Departmental Identification Cards

  • Any individual who loses the departmental identification (IDInstitutions Division (see DAI)) card shall immediately report the loss by completing a Lost IDInstitutions Division (see DAI) Badge form located on the Human Resources (HRHuman Resources) intranet site http://intranet/ADM/DSS/hr/ops/Pages/Lost%20ID.aspx.

    • The completed form is to be signed by a supervisor and submitted to the
      hiring authority (HA).

    • The HA will review the form and provide the signed Lost IDInstitutions Division (see DAI) form to the
      Lost IDInstitutions Division (see DAI) Coordinator.

  • Each program is allowed two Lost IDInstitutions Division (see DAI) Coordinators.

    • The staff identified by the HA as Lost IDInstitutions Division (see DAI) Coordinators will be provided access to the Department’s HRHuman Resources intranet Lost IDInstitutions Division (see DAI) site.

    • If there are no Lost IDInstitutions Division (see DAI) Coordinators, the HA shall click the “Request access to post Lost IDInstitutions Division (see DAI)” link located on the Department’s HRHuman Resources intranet Lost IDInstitutions Division (see DAI) page. The link will open an email to CDCRCalifornia Department of Corrections and Rehabilitation HRHuman Resources Personnel Customer Service.

  • The facility, parole region, or division Lost IDInstitutions Division (see DAI) Coordinator, or HA if no Lost IDInstitutions Division (see DAI) Coordinators are identified, shall:

    • Post the information on the Department’s HRHuman Resources intranet Lost IDInstitutions Division (see DAI) page list to advise all departmental staff that the IDInstitutions Division (see DAI) card has been reported lost.

    • Send a notice to all other facilities advising of the loss. This notice shall include the individual’s name and type of identification card, and any information relevant to possible misuse of the card.

      • The notice shall specify: “Should anyone attempt to use the lost identification card to gain entrance to your facility, please confiscate the card and notify this facility/region/division at once.”

      • A copy of the notice that was sent to all facilities shall be included with the
        CDCRCalifornia Department of Corrections and Rehabilitation Form 894-A when requesting a replacement IDInstitutions Division (see DAI) card.

  • If a card is mutilated, lost, or stolen, a new card shall be issued immediately.
