54090.4.1 Form Availability

• The Department shall ensure that the CDCRCalifornia Department of Corrections and Rehabilitation Form 22, Inmate/Parolee Request for Interview, Item or Service, is readily available to inmates and parolees. This form shall be available in all inmate housing units, general or segregated; all institutional libraries; any facility under the Department’s jurisdiction, whether residential or medical, where inmates are required to remain more than 24 hours; and all parole field offices.

• This form is printed on NCRNotice of Change to Regulations (no-carbon-required) paper, which facilitates:

  • A record of the date the form was first presented to staff, and the date of each staff response.

  • A record of the nature of the request and any subsequent efforts to address it.
