  • The counselor shall prepare a CDC Form 841 upon the return to CDCRCalifornia Department of Corrections and Rehabilitation custody from parole of a PVRTCParole Violator Return To Custody or a PVWNTParole Violator Returned With New Term. Also prepare a CDC Form 816, Reception Center Readmission Summary, for the PVRTCParole Violator Return To Custody or PVWNTParole Violator Returned With New Term with the following exceptions.

  • Prepare a CDC Form 841 and an ISRSInstitutional Staff Recommendation Summary for a PVWNTParole Violator Returned With New Term returning with a new Life term.

  • Prepare only a CDC Form 841 if parole is lawfully revoked by the BPHBoard of Parole Hearings (formerly Board of Prison Terms) because an inmate refused to sign conditions of parole. Preparation of a CDC Form 816 is not required if the inmate is not physically paroled to the community, the inmate’s status shows that he/she has been paroled, revoked, and returned the same day.

  • Once a PVRTCParole Violator Return To Custody or PVWNTParole Violator Returned With New Term is scored on a CDC Form 841, each subsequent readmission period shall be captured on a separate CDC Form 841. The counselor prepares a CDC Form 841 each time a parole violator returns to the RCReception Centers. For example, if a PVRTCParole Violator Return To Custody paroled from the RCReception Centers prior to the preparation of a CDC Form 841 for that revocation, prepare a CDC Form 841 to address that RTCReturn To Custody period as well as a separate CDC Form 841 to address the current RTCReturn To Custody.

  • A CDC Form 841 shall not be prepared for a parolee pending revocation proceedings. Do not prepare a CDC Form 841 if the parole violator is continued on parole (COPContinue On Parole).

  • A CDC Form 841 shall not be prepared for a parolee who is returned to custody as a “non-revoked” parolee, for example a SATCU placement.

  • A CDC Form 841 shall not be prepared for a parolee who is returned to custody for “psych attention” only. A parolee revoked by the BPHBoard of Parole Hearings (formerly Board of Prison Terms) for psych attention is in custody for psychiatric treatment, but continues his or her parole period uninterrupted.
